Documents Required for Medical Services
-
To be reimbursed by CAS, you need your ID card, the referral form, and proof of insurance status; this can be your health card, a CAS certificate, or a document confirming payment of contributions. Patients who have been issued a health card must bring it; patients who have not been issued a health card do not need to bring anything, as their insured status will be verified in the CAS system. If a patient was issued a health card but has lost it, they will need a CAS certificate to replace the card.
-
You will need a referral from a specialist, your ID card, and proof of insurance coverage (your health insurance card, if issued). It is recommended that the patient bring a file containing all medical letters and the results of previous MRI scans, if available.
